i've had an issue with mine before, due the heatin/cooling cycle of the fluid it must have tightened right up. i had to use a pair of large pliers to get it undone.
now i just make sure it's not super tight when checking the fluid level.

The fluid in the system is at atmospheric pressure and you dont need to tighten the cap. Never had a problem as I dont do it up too tight.
Try a smear of nickle antisieze on the threads, I have to do this with my PWR aluminium power steer reservoir or it's impossible to undo by hand.
